Job Description - Case Coordinator

This position will work in collaboration with patients, caregivers and the clinical team. Primary responsibility of this role is to determine patient’s needs and work closely with the care team to evaluate interventions. This position will functionally report to Physician and administratively report to Patient Liaison Manager.

Roles and responsibilities:

• Identify, gather and track medical records needed for case information accurately
• Under the supervision of a physician, triaging patient medical records to select appropriate specialist to provide patient with remote medical second opinion via video or phone call
• Facilitate care and treatment via scheduling and information sharing
• Coordination of second opinion consultation between patient and multidisciplinary care team from the affiliated hospital
• Advocate for patients with advice and information regarding treatment options and clinical status so that timely informed decision can be made
• Develop effective working relations with care team throughout the case management process
• Responsible for delivery of seamless continuum of care
• Responsible for maintaining regular contact with patients to provide ongoing support and counselling to assist in achieving goals
• Ensure structured clinical pathways are designed to support implementation of clinical guidelines and protocols
• Adhere to professional standards as outlined by protocols, rules and regulations

Qualification and training:

• MBBS/BDS/BAMS/BHMS
• Knowledge of American standard clinical pathways
• Effective communication and customer service skills
• Excellent interpersonal skills
• Problem solving skills and ability to multi-task
• Be highly empathetic
